What to bring to your first visit

With these, your visit starts without waiting or extra paperwork.

Your plan card
Your vision plan card and your medical insurance card, if you have both.
An ID
With your full name.
Your current eyewear
The glasses and contact lenses you wear, and your last prescription if you have it.
Your medicines
A list of the ones you take, so the doctor has it on hand.

Before your visit

Call or leave your number
Reina books your visit and tells you how it will be billed.
Fill in your forms
You can do it at home, on our office's forms page, or here 10 minutes before your visit.
Open the forms
Arrive and come in
Parking right by the door. The front desk greets you in Spanish or English.
